Reinstall that mod and look at the moddescription how to uninstall them. it's possible a mod unequipped it from your character. Use player.additem 0011bacb 1 to add it to your inventory if it was removed.The vanilla pip boy is an equipable that just doesn't show in your inventory; mods like pip pad that remove it from when they take away that item. Look up the ref ID for the vanilla pip boy and type player.equipitem XXXXXXXX into the game console. That may fix the issue

Most games in the fallout series don't disable achievements when using debug cmds. I jumped my level up to 100 using the console, and it didn't disable achievements. It's not achievements, it's Survival Mode, but it wasn't long before mods enabled console in Survival Mode, because of game-breaking bugs like this one.This is not the chameleon bug or invisible character issue.
